Adult Social Care offers a range of teams and services to support local people in living fulfilling lives. Our teams include social care practitioners, social workers, and occupational therapists.

We work closely with health services, including primary care, mental health, and learning disability teams, as well as our Housing and Community’s directorate and the Community and Voluntary sector.

We are adopting the three conversations model to ensure our practice is strengths-based, relational, and personalised. 

Our teams include

  • Long-Term Conditions teams, who support adults in the community and at local hospitals
  • Integrated Learning Disability and Mental Health teams
  • a Preparing for Adulthood team, which supports young people aged 18 to 25
  • specialized teams for Adults with Autism, Approved Mental Health Professionals, and more

  • continued professional development (CPD) with training and reflective practice, in partnership with Bournemouth University
